The information of most use on referral is:

• History of dental trauma/marked dental pathology. In particular if you view a tooth as being of poor long term prognosis.
• The Patient's complaint.
• Relevant Medical History.
• An accurate contact address and telephone number.

The PCT will shortly circulate a proforma referral form for the use in all orthodontic referrals within Shropshire. There should still be the opportunity to provide the above information on this form.
